Personal Advisor - Leaving Care Team
Job Description:
Contract to start ASAP until 14-09-26 with a possible extension.
Job Purpose
To provide practical, emotional, and professional support to young people leaving care, helping them transition successfully into independent adulthood.
The Personal Advisor will work closely with care leavers aged 16–25 to promote positive outcomes in education, employment, training, accommodation, health, and wellbeing in line with statutory responsibilities under the Children (Leaving Care) Act.
Responsibilities
- Act as a designated Personal Advisor for care leavers within the Leaving Care Team.
- Develop, implement, and regularly review Pathway Plans tailored to individual young people's needs and aspirations.
- Build positive and supportive relationships with young people to encourage engagement and independence.
- Provide advice and guidance regarding housing, budgeting, benefits, education, employment, training, and life skills.
- Support young people to access appropriate services including health, mental health, substance misuse, and community resources.
- Undertake regular visits and maintain accurate, up-to-date case records in line with council procedures and statutory requirements.
- Work collaboratively with social workers, housing providers, education services, and external agencies to coordinate effective support plans.
- Advocate on behalf of young people to ensure their voice is heard and their rights are promoted.
- Safeguard and promote the welfare of vulnerable young people at all times.
- Participate in team meetings, supervision sessions, and training activities
