Job Description:
Contract to start ASAP
Duration: 8 weeks with a possible extension.
Responsibilities
Caseload Management: Hold responsibility for a defined caseload of care leavers, ensuring timely assessments, planning, and interventions.
Pathway Planning: Develop and review aspirational Pathway Plans covering accommodation, education, employment, health, and financial independence.
Practical Support: Assist with budgeting, bills, tenancy management, shopping, and life-skills training.
Advocacy & Coordination: Work collaboratively with multi-agency partners (housing, education, health, benefits) to secure positive outcomes.
Emotional Support: Provide trauma-informed, relationship-based support to promote resilience and wellbeing.
Safeguarding: Identify and manage risks, ensuring compliance with statutory duties and safeguarding protocols.
Record Keeping: Maintain accurate case notes and reports in line with local authority standards.
Duty Cover: Participate in duty rota to respond to urgent needs and crisis situations.
Full UK driving licence and access to a vehicle
