Duration: 12 weeks with a possible extension.

JOB PURPOSE

The role is to provide a service to children, young people, their families or carers within their placements or community, which includes direct work with children and young people.

Support Workers assist and support qualified officers in the discharge of the statutory obligations of the authority in relation to children and young people who may be looked after, or the subject of a care and support plan.

Case management responsibility will be allocated in appropriate circumstances for children and young people who have care and support needs.

This will include assessment, planning and review for those children and young people.

The specific nature of the role will vary according to the team.

Provide direct support as part of the care and support plan based on assessed need of children and young people including children and young people who are Looked After, or who may be the subject of other statutory or court related order(s).

Undertake assessments of children and young people in need of care and support, where required, under the direction of a team manager, or senior practitioner.

Represent the authority and liaise with partner agencies both statutory and voluntary in the provision of services for children, young people, and their families.
