Specialist SEN and Disability Outreach Worker
Job Description:
Contract to start ASAP until 21-09-26 with a possible extension.
Job Purpose
To work as a specialist SEN and Disability outreach worker, within Enfield's Service for Disabled Children.
To build effective partnerships with parents/carers, professionals, and service providers to ensure positive outcomes.
To support children and young people and their families to access effective and person centrered short breaks and community activities.
To provide advice, information and guidance to children and young people with SEN and disabilities and their families.
To undertake short break assessments and reviews to ensure that the right support is provided and continues to meet identified needs.
To work alongside the SEND Transition Manager in ensuring that transition pathways are progressed in a timely way and to contribute to Moving On events.
To ensure that the participation and views of children and young people with SEN and Disabilities inform service development
To undertake the lead professional role for children and young people with SEND and disabilities when appropriate.
To work alongside Social Workers to ensure that the support plan is progressed
