Duration: 12 weeks with a possible extension.
Job Purpose
The EHCP Co-ordinator is responsible for coordinating the statutory Education, Health and Care (EHC) needs assessment and review process, ensuring that Education, Health and Care Plans (EHCPs) are completed and maintained in accordance with relevant legislation and statutory timescales.
The postholder will work collaboratively with families, schools, educational settings, health professionals, social care teams, and other stakeholders to deliver high-quality, person-centred outcomes for children and young people with Special Educational Needs and Disabilities (SEND).
Confident and experienced in the coordination of EHCP Annual Reviews.
Experience of EOTAS, CME and EHE.
Capita experience desirable.
Specifically to work within an Annual Review backlog recovery team.
Responsibilities
Manage a caseload of Education, Health and Care Plans (EHCPs) from assessment through to annual review and amendments.
Coordinate the statutory EHC needs assessment process, ensuring compliance with the Children and Families Act 2014, the SEND Code of Practice, and local authority policies.
Liaise with parents, carers, educational settings, health professionals, social care teams, and other agencies to gather relevant information and evidence.
Draft, amend, and issue EHCPs within statutory timescales.
Organise and coordinate annual reviews, ensuring recommendations are implemented promptly.
Provide advice and guidance to families and professionals regarding the EHCP process and SEND legislation.
Monitor case progress, identify risks to statutory deadlines, and take proactive action to resolve delays.
Maintain accurate and up-to-date records using the council's case management systems.
Prepare reports, correspondence, and documentation to support decision-making and panel meetings.
Attend multi-agency meetings, case conferences, mediation, and tribunal proceedings where required.
