Job Purpose

Wirral MBC is seeking a committed and supportive Personal Advisor to work with young people leaving care, helping them transition successfully into independent adulthood.

The role involves providing practical, emotional, and social support to care leavers to improve outcomes in areas such as accommodation, education, employment, training, health, and wellbeing.

Responsibilities

Provide ongoing advice, guidance, and support to young people leaving care.

Build positive and trusting relationships with care leavers aged 16–25.

Assist young people in developing independent living skills.

Develop, implement, and review Pathway Plans in line with statutory requirements.

Support young people to access education, employment, training, housing, and health services.

Conduct regular visits, welfare checks, and risk assessments.

Work collaboratively with social workers, housing providers, education services, and partner agencies.

Advocate on behalf of young people to ensure their needs and rights are met.

Maintain accurate case records, reports, and documentation.

Promote safeguarding, equality, diversity, and inclusion in all aspects of work.
