Duration: 12 weeks with a possible extension.

Job Purpose

To provide intensive, person-centred support to people living with dementia and their carers, enabling them to remain safely in their own homes and communities.

The role focuses on crisis intervention, prevention of hospital admissions, and improving quality of life.

Responsibilities

Provide specialist outreach support to individuals with dementia and their families.

Deliver intensive, short-term interventions to prevent placement breakdown and hospital admission.

Undertake holistic assessments of service users needs and risks.

Develop, implement, and review personalised care and support plans.

Work collaboratively with health professionals, social workers, and community services.

Support carers with guidance, training, and emotional support.

Respond to crisis situations and escalating behaviours.

Promote independence, dignity, and wellbeing.

Maintain accurate and up-to-date case records and reports.

Candidates must have experience of supporting people living with dementia*

Enhanced DBS with Barred List for working with Adults

Please ensure workers have all the Mandatory Training listed

Confirmation that the candidate can demonstrate completion of the following mandatory training:

Full UK driving licence
