Job Description:

Contract to start ASAP until 18-09-26 with a possible extension.

Responsibilities:

To provide short-term enabling support in individual's homes for new and existing service users both in the community and after hospital discharge, in order to optimise independent living.

To assist in safely promoting independence and supporting users through reablement to maximise their independence for as long as possible.

To help to establish the appropriate level of support for individuals with on-going needs.

To enable, motivate and assist service-users in their activities of daily living, with an emphasis on enhancing independence.

To support service-users with their life-style choices and in achieving agreed goals

To actively participate in the on-going monitoring and review of the reablement process, reporting regularly to the Manager in order that the appropriate level of service is provided

To report to the manager on any change in need.

To comply with Enfield' Council's Electronic Monitoring System.

To adhere to the guidance issued on the use of Enfield Council's mobile telephones or other technology.

To administer medication in accordance with the Enfield Council's Policy for Safe Practice in Medication Administration.

To comply with all service policies and procedures relating to quality service provision.

To keep accurate written records of the support given to the service-user at every visit, in accordance with the Council's Recording Policy.
