Job Description:

Contract to start ASAP until 08-05-26 with a possible extension.

Responsibilities:

To undertake initial assessments within the area of specialism.

To manage, monitor and review care plans involving users and carers and liaising with other workers anc agencies as appropriate.

To work at all times within the council's Financial
Regulations

To be aware of the resources available within the specialist area, including other sources of funding so ters within budgetary resources of the or our service

To make use of the management information systems, including information technology, in the recording retrieval and analysis of information as required by the department.

To be managed and receive supervision from the immediate line manager and to be assessed against agreed standards and targets.

Carry out all accountabilities in compliance with the Council's Policies and Procedures

Any other duties reasonably requested by management
