Job Description:

Contract to start ASAP until 18-05-26 with a possible extension.

Job Purpose

To provide high-quality case management services to residents, ensuring effective assessment, support, and coordination of services.

The Case Manager will work closely with internal departments and external agencies to deliver timely, person-centred solutions that improve customer outcomes.

Responsibilities

Manage a caseload of clients, assessing needs and developing tailored support plans.

Act as the main point of contact for service users, providing guidance and ongoing support.

Coordinate multi-agency services including housing, health, social care, and community organisations.

Monitor progress, review cases regularly, and update support plans as required.

Maintain accurate and confidential case records in line with data protection policies.

Prepare reports, case summaries, and recommendations for management and partner agencies.

Support clients in accessing appropriate services, benefits, and housing options.

Identify risks and safeguarding concerns and take appropriate action in line with policies.

Attend case conferences, reviews, and professional meetings.

Ensure compliance with local authority procedures, legislation, and best practice standards.
